His solipsism is sui generis. He is psychologically remarkable in\u00a0his capacity to manufacture and continuously assert falsehood in the\u00a0apparent absence of psychosis.<\/p>\n<\/blockquote>\n<p>Not unexpected the advice was rejected by basically all medical professionals. Commentaries are raging in <a href=\"https:\/\/www.nature.com\/articles\/d41586-025-02876-1\">Nature<\/a>, <a href=\"https:\/\/doi.org\/10.1136\/bmj.r2025\">BMJ<\/a> ,\u00a0<a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(25)02001-X\/fulltext\">The Lancet<\/a>, <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2839562\">JAMA<\/a> and even the <a href=\"https:\/\/www.who.int\/news\/item\/24-09-2025-who-statement-on-autism-related-issues\">WHO<\/a>. <!--more--><\/p>\n<blockquote><p>The World Health Organization (WHO) emphasizes that there is currently no conclusive scientific evidence confirming a possible link between autism and use of acetaminophen (also known as paracetamol) during pregnancy.<\/p><\/blockquote>\n<p>Most recently <a href=\"https:\/\/www.washingtonpost.com\/opinions\/2025\/10\/07\/surgeons-general-rfk-jr-robert-kennedy\/\">six surgeons general<\/a> warned even about RFK jr.<\/p>\n<blockquote>\n<p class=\"p1\">Yet Kennedy continues to ignore science and the public\u2019s wishes. Most recently, HHS proposed new warning labels on products containing acetaminophen (Tylenol), citing a supposed link between prenatal use\u00a0and autism. This move has been widely condemned by the scientific and\u00a0medical communities, who have pointed out that the available research\u00a0is inconclusive and insufficient to justify such a warning. In an\u00a0extraordinary and unprecedented response, the American Academy ofPediatrics, the American College of Obstetricians and Gynecologists and other leading health organizations issued public guidance urging\u00a0physicians and patients to disregard HHS\u2019s recommendation. Instead of\u00a0helping pregnant women make informed decisions during a critical\u00a0period in their lives, Kennedy\u2019s decisions risk causing confusion, fear and harm.\u00a0 Rather than combating the rapid spread of health misinformation with\u00a0facts and clarity, Kennedy is amplifying it. The consequences aren\u2019t\u00a0abstract. They are measured in lives lost, disease outbreaks and an\u00a0erosion of public trust that will take years to rebuild.<\/p>\n<\/blockquote>\n<p>So let&#8217;s have a look<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40804730\/\"> at the main paper<\/a> that caused this storm.\u00a0Who is writing what and why?<\/p>\n<p>The first author DP is a\u00a0<a href=\"https:\/\/www.instagram.com\/dubaieye1038fm\/reel\/DN-FDJ9kbyB\/\">MD<\/a> with a publication record at <a href=\"https:\/\/scholar.google.com\/citations?user=KTj967gAAAAJ&amp;hl=en\">Scholar<\/a> showing an IF of 23 over a period of 16 years. He <a href=\"https:\/\/www.linkedin.com\/in\/diddier-prada-89051435\">proudly shares at Linkedin <\/a>that his research is even discussed at <a href=\"https:\/\/www.foxnews.com\/video\/6377270580112\">FOX News<\/a> &#8230; The second author BR is a German MD, known for previous\u00a0 dubious claims <a href=\"https:\/\/di.aerzteblatt.de\/int\/archive\/article\/213561\">that I have discussed at Deutsche \u00c4rzteblatt<\/a>.\u00a0The third author AZB has a BS in Industrial Engineering and Epidemiology and first authored just 4 papers in the last 12 years including a &#8220;consensus statement&#8221; about paracetamol that was heavenly criticized (<a href=\"https:\/\/www.nature.com\/articles\/s41574-021-00606-x\">ref<\/a>, <a href=\"https:\/\/www.nature.com\/articles\/s41574-021-00605-y\">ref)<\/a>. The last author Andrea Baccarelli s an Italian MD\/PhD. The <a href=\"https:\/\/www.thetimes.com\/article\/aa7fff5a-d267-440f-a544-a3cd75c33416\">Times<\/a> described him<\/p>\n<blockquote>\n<p class=\"p1\">Scientist behind Trump\u2019 s Tylenol claims was paid $150K to give evidence against drug maker.\u00a0 The Harvard academic Andrea Baccarelli gave an \u2018unreliable\u2019 testimony on the links between autism and paracetamol, and produced research that raises \u2018serious concerns about bias\u2019.<\/p>\n<\/blockquote>\n<p>The <a href=\"https:\/\/law.justia.com\/cases\/federal\/district-courts\/new-york\/nysdce\/1:2022mc03043\/588200\/99\/\">court decision<\/a> did not leave any doubt about his earlier testimony<\/p>\n<blockquote><p>&#8230; Dr. Baccarelli also disregards relevant reviews of epidemiological studies conducted by medical and governmental associations. He does not address the FDA\u2019s repeated conclusion that the epidemiological evidence does not support his opinions, other than to note his disagreement with that conclusion. Nor does he grapple with the contrary conclusions of the American College of Obstetricians and Gynecologists, the Society of Obstetricians and Gynaecologists of Canada, or the European Network of Teratology Information Services. This \u201crejection of a conclusion that could not be more relevant to his opinions is alarming.\u201d In sum, Dr. Baccarelli failed to sufficiently explain the appropriateness of conducting a single Bradford Hill analysis for NDDs which included ASD and ADHD, selectively analyzed the consistency of the literature and the issue of genetic confounding, repeatedly pressed conclusions that study authors were not willing to make, and disregarded studies that do not support his opinion due to limitations that he did not view as disqualifying in studies that did support his opinion. Together, these deficiencies demonstrate that his opinion does not \u201creflect[] a reliable application of the principles and methods to the facts of the case.\u201d<\/p><\/blockquote>\n<p>So the &#8220;who&#8221; question could be easily answered. The two authors with the lowest qualification (AB and DP) selected the studies and assigned the scores while there was no expert in pharmacoepidemiology or any author involved with more than casual experience in meta-analysis or autism research. The answer to the &#8220;why&#8221; question is also be obvious &#8211; Baccharelli would have to pay back his $150K if he would publish now the opposite.<\/p>\n<p>We can therefore move on\u00a0 to the more important &#8220;what&#8221; is in this &#8220;rigorous review&#8221;.<\/p>\n<p>Unfortunately I am already stuck at the abstract. It says that 27 positive, 9 null and 4 negative studies were included (in total 40) but continues with 46 studies. The included chart has even two more internal contradictions that can be found immediately. Can you find them? Some studies were missed during grading and then errors were back propagated even to the abstract.<\/p>\n<p>Studies have also been wrongly reported. Already the first study in table 1 has a wrong CI (it is\u00a0 of course 1.05-1.10). The second study gives a wrong sample size (the 31,156 are discordant on exposure, not on outcome!) although I am not going to check this further.<\/p>\n<p><a href=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27.jpg\" rel=\"key\" data-rel=\"key-image-3\" data-rl_title=\"\" data-rl_caption=\"\" title=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-25698 size-medium alignnone\" src=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27-620x311.jpg\" alt=\"\" width=\"620\" height=\"311\" srcset=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27-620x311.jpg 620w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27-996x500.jpg 996w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27-768x386.jpg 768w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2025\/10\/Bildschirmfoto-2025-10-11-um-11.32.27.jpg 1450w\" sizes=\"auto, (max-width: 620px) 100vw, 620px\" \/><\/a><\/p>\n<p>What about the confounder reported? <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2839562\">Their effect is not equal<\/a> according to previous research.<\/p>\n<blockquote>\n<p class=\"p1\">For neurodevelopmental disorders especially, they are highly heritable, and genetics comprises a substantial proportion of their cause. So studies that don\u2019t take into account genetics may come to different conclusions than studies that do take into account genetics.<\/p>\n<\/blockquote>\n<p>But how did the authors define the bias classes? Just by eyeballing?\u00a0 Or any SOP? Why <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2839562\" target=\"_blank\" rel=\"noopener\">has the most important confounder<\/a> even be omitted from the analysis?<\/p>\n<blockquote><p>Studies were rated as higher risk of bias (score of 3 or 4) if they lacked adjustment for key confounders, such as maternal age, chronic illness, socioeconomic status, smoking, alcohol use, or clinical indications for acetaminophen use (e.g., fever or infection).<\/p><\/blockquote>\n<p>One more thing: The authors describe basically no bias in autism outcome which is in <a href=\"https:\/\/www.nature.com\/articles\/s41586-025-09542-6\" target=\"_blank\" rel=\"noopener\">contrast to more recent research<\/a> that show a mixture of different age-dependent polygenic traits. Unfortunately age of diagnosis was never considered by the authors.<\/p>\n<p>If exposure and and outcome in ASD studies are so heterogenous, we can believe in one or the other study. But there the overall\u00a0 conclusion is not supported that &#8220;immediate steps should be taken to advice pregnant women to limit acetaminophen consumption&#8221;. Also the primary Environmental Health reviewer has noticed this framing. The authors politely replied that<\/p>\n<blockquote><p>We acknowledge the need for a balanced presentation and recognize that our initial framing may have appeared to favor positive results.<\/p><\/blockquote>\n<p>but simply kept their narrative construction to shape the <a href=\"https:\/\/www.thetimes.com\/article\/aa7fff5a-d267-440f-a544-a3cd75c33416\" target=\"_blank\" rel=\"noopener\">political discourse<\/a> at the cost of scientific integrity.<\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"text-decoration: underline;\">Postscript 1<\/span><\/p>\n<p>Bonus point for the 4 authors, the 3 assistants in the acknowledgment, the 2 reviewer and the 1 editor for leaving an internal bold marked comment in the PDF on page 12.<\/p>\n<p><span style=\"text-decoration: underline;\">Postscript 2<\/span><\/p>\n<p><a href=\"https:\/\/pubpeer.com\/publications\/06BC8C35A8DE4D2B1C3AAF8B044610\">My Pubpeer review<\/a> has been submitted to Philippe Grandjean (editor of\u00a0 Envir Health) on 9Oct25 with no response.
