{"id":9577,"date":"2018-02-20T16:42:32","date_gmt":"2018-02-20T15:42:32","guid":{"rendered":"http:\/\/www.wjst.de\/blog\/?p=9577"},"modified":"2023-02-16T13:26:59","modified_gmt":"2023-02-16T11:26:59","slug":"allergy-risk-by-baby-food","status":"publish","type":"post","link":"https:\/\/www.wjst.de\/blog\/sciencesurf\/2018\/02\/allergy-risk-by-baby-food\/","title":{"rendered":"Allergy risk by baby food"},"content":{"rendered":"<p>A new analysis of the GINI-LISA study published this month in\u00a0<span role=\"menubar\"><a title=\"Pediatric allergy and immunology : official publication of the European Society of Pediatric Allergy and Immunology.\" role=\"menuitem\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28881061#\" aria-expanded=\"false\" aria-haspopup=\"true\">Pediatr Allergy Immunol<\/a><\/span>\u00a0has an interesting plot showing the eczema incidence in the first 15 years of life. Eczema is a major allergy symptom.<\/p>\n<p>On the right side (FIG1C), we find the untreated group, and on the left (FIG1A) we find the intervention group. Overlying both figures with Photoshop gives an interesting perspective.<\/p>\n<p><a href=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini.jpg\" data-rel=\"key-image-0\" data-rl_title=\"\" data-rl_caption=\"\" title=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-9670\" src=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini-620x502.jpg\" alt=\"\" width=\"620\" height=\"502\" srcset=\"https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini-620x502.jpg 620w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini-768x622.jpg 768w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini-618x500.jpg 618w, https:\/\/www.wjst.de\/blog\/wp-content\/uploads\/2018\/02\/Gini.jpg 893w\" sizes=\"auto, (max-width: 620px) 100vw, 620px\" \/><\/a><\/p>\n<p>When we use the lower opaque double line as reference, all types of baby feeding \u00a0give an increased eczema risk. To be more comparable, the opaque upper two lines should be used, which will result in zero effect size.<\/p>\n<p>The question is: Why is breast feeding \/ breast milk (BF) a risk factor in the interventional (left figure) \u00a0but not in the observational arm (right figure)?<\/p>\n<ol>\n<li>In accordance with national and international guidelines full BF was recommended for 4 months in the interventional group. &#8220;Only in case of insufficient BF, children were randomized to one of the baby foods&#8221;. &#8220;Insufficient BF&#8221; is not clearly defined and\u00a0<a href=\"http:\/\/www.spiegel.de\/gesundheit\/schwangerschaft\/stillen-warum-viele-frauen-frueher-aufhoeren-als-empfohlen-a-1169895.html\">the reasons seem to be complex<\/a>.<\/li>\n<li>The <a href=\"https:\/\/www.kindergesundheit-info.de\/themen\/ernaehrung\/stillen\/stillen\/\">general pediatric recommendation<\/a>\u00a0at the time of the study was that BF is allergy-preventive and that allergic children should be BF as long as possible.<\/li>\n<li>BF in the interventional arm therefore is a self-selected biased group primarily by allergic mothers and children with unclear criteria. The authors acknowledge in the discussion &#8220;Even adjustment for the potential confounders does not eliminate the phenomenon of reverse causality. Particular early eczema occurring in the first months of life may influence the mother\u2019s decision regarding full breastfeeding versus formula supplementation.&#8221;<\/li>\n<li>BF is a complex chemical mixture that even varies over time &#8211; there is\u00a0is a <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0022347688800544\">long-standing controversy on the effect of BF on allergy <\/a>\u00a0with many negative, null and positive associations to allergy.\u00a0From this discussion we can assume, that there is probably no direct effect of BF on allergy development. More likely BF is\u00a0a proxy for something else &#8211; setting up a further biological argument that the interventional BF group should NOT be used as a \u00a0reference.<\/li>\n<\/ol>\n<p><span style=\"text-decoration: underline;\">Children who receive &#8220;antiallergic&#8221; formula food will not have any benefit<\/span> which is in accordance with a <a href=\"http:\/\/www.bmj.com\/content\/352\/bmj.i974\">recent review in the BMJ<\/a>.\u00a0Nevertheless\u00a0Nestl\u00e9 Beba Pro HA is further advertising as &#8220;reducing allergy risk to milk protein&#8221;, Hipp HA Combobiotik as &#8220;reducing allergy risk by strong protein cleavage&#8221;, Nutramigen 1 LGG as &#8220;extensively hydrolysed, hypoallergenic formula &#8230; suitable for babies with &#8230; allergy&#8221;.<\/p>\n<p>&nbsp;<\/p>\n<p><u>Postscriptum<\/u><\/p>\n<p>The revised blog entry has <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29637629\/\">been published as a letter<\/a> to PAI.
